In order to collect data about amphibian diseases, and
therefore contribute to vital research, we're asking anyone who
has witnessed an outbreak of ranavirus to please fill in a
questionnaire.
The only way we can hear about new cases of disease is if you
let us know. You're data provides crucial evidence for how the
disease is spreading.
If you don't want to submit your contact details you don't have
to but you will need to include your postcode, as without this
the record is virtually useless. If you do include your contact
details we will not pass these on to third parties; if you would
prefer them not to be passed on to our research partners with
the rest of the information, please tick the box at the end of
the form.
The information feeds through to our research partners who use
it to find out more about the disease. If you have any questions
regarding the outbreak please contact us directly rather than
including them on the form as you will get a much quicker
response. We will try to keep you up to date with current
findings and may get in touch if a sample is required for
analysis and. Find out more about
submitting a sample.
No. This form is specifically to record incidences of ranavirus.
If you've found frogs with other symptoms or at other times of

amphibian deaths.
