Encourage all volunteers to record data, either by carrying a
copy of the recording form with them or by making notes and
filling in the form later (either a paper copy or online). All
data feeds into national monitoring projects and gives us a good
idea as to how the UK's common toads are faring as a whole. It
can also give your Patrollers a boost to calculate how many
toads they've helped.
The form is straightforward - record the date, the number of
toads helped and those seen killed, plus any other amphibians.
Make a note of the times you were patrolling each night and how
many volunteers were patrolling. If you have any additional
notes - for example 'good weather but no toads' or 'more traffic
than usual' - please add these into the comments column. If
possible, please provide an estimate of the number of vehicles
seen on the road per hour. Finally make sure the site name and
ID number are at the top of the form along with the name of the
person who made those records and a contact number or email
address for them.

or you can use one of the following online forms:
